5sfe camshaft timing marks

Rebuilt my 5sfe now having problems - Toyota Nation Forum : Toyota.

Lower left of picture, the cylinder about 1/2" away from the cam sprocket. 5SFE cam sproket timing marks. The sprocket itself is NOT on the correct timing mark.

Not sure what happened, but the cam timing or something else is totally off.. I don't have the alignment procedure available for a 5sfe engine, but .. I would check the timing belt alignment marks, and see where the cam falls.
Which of these three holes on the intake camshaft is the timing mark? they need to match up to the intake two identical timing marks that are 5.
